Agent marks a console step done	Human task claimed by the agent	Reject that; run `ack-human` yourself with redacted evidence
Design / UI work stuck	Design evidence path not available	Install the design evidence tool your plan includes, or keep UI out of the critical path
Wrong kind of work (plan vs UI vs fix)	Ambiguous request	Reword with behaviour you want; or prefix `~` to skip Naro for a clarifying question
